As most of you know, Spivack Vision Center® (SVC) and Colorado Laser Surgeons (CLS) merged over two years ago. We have endeavored to create the same culture of optometric co-management at SVC that existed with CLS. Merging two cultures has been more challenging than was anticipated. After two years, we feel we have accomplished that goal.
Eye Allergy Overview
Springtime in Colorado is well on its way and along with that typi- cally comes seasonal allergies. We are already seeing several allergic conjunctivitis cases present in our clinics. With new medications available on the market, an overall update on allergic conjunctivitis treatment may be helpful.
Etiology:
- Type I hypersensitivity reaction to specific airborne allergens. Conjunctival mast cells degranulation releases histamine and proinflammatory mediators into the tear film and tissues. This causes dilation of conjunctival vessels leading to red eyes, overall edema of tissues, and significant itch.
- Seasonal Allergic Conjuctivitis – related to hay fever. This is caused by seasonal allergens such as tree pollen, weeds, grasses, and fungal spores
Common Medications and their Eye-Related Side Effects
When prescribing systemic medications keep in mind that there can be ocular side effects. Below, I have listed multiple medications and drug classes that are commonly prescribed.
Systemic Hypertension Medications:
Beta Blockers: atenolol, propranolol, metaprolol- The primary ocular side effect with using this medication is dry eye with complaints of gritty eyes, irritation, foreign-body sensation and photophobia. These symptoms are due to a reduced tear production and dry eye syndrome. If patients have these complaints advise the patient to use artificial tears more frequently and/or consider prescribing Restasis, a medicated eye drop for dry eye treatment.
Corneal Ulcers: What you need to know.
A corneal ulcer is lesion on the cornea where the surface epithelium has eroded or been compromised. Subsequent infection that proliferates can result in vision reduction, scarring and ultimate vision loss yet could advance to the point Where loss of the eye is possible. Fortunately today, We have excellent topical medication (rarely is oral medication of any value) that can limit the scarring and reduce the potential for loss if the condition is treated aggressively and quickly.
Causes of corneal ulcers:
Tiny tears to the corneal surface may become infected and lead to corneal ulcers. These tears can come from direct trauma by scratches, metallic or glass particles striking the cornea or stressing of the surface cells from contact lenses.

Dr. Zamora received his doctorate of medicine from Tulane University School of Medicine. He completed his internship and ophthalmology residency at Boston University.
Jack subsequently completed his Oculoplastics fellowship at Boston University becoming an assistant professor at the university and was named as one of Boston University’s premier surgical instructors. Dr. Zamora is a member of numerous national medical societies.
Cataract Surgery Statistics
The doctrine of informed consent relates back to English common law. Common law refers to a judge using prior case precedents to determine a legal course as opposed to statutes which are legislated direction. Beginning in the late 18th century, doctors were charged with the tort of battery if they had not gained the consent of their patients prior to performing a surgery. A tort if s a civil wrong intentional or not, not in the context of a contract or statute. Tort law terms, including “standard of care” and “negligence” are front and center in today’s medical malpractice environment.

When I graduated from Tufts Medical School in Boston, and came to Denver, I never dreamed of the changes that would occur in Ophthalmology and in my career. Like all young residents, I saw myself doing primary care and cataract surgery for the majority of my life. I had never even heard of the work going on in South America and the Soviet Union that would create a sea-change in eyecare and in society.
